Information for Physicians and Health Care Professionals
Take Charge of Your Diabetes is an evidence-based program developed by Stanford University's Patient Education Research Center. Participant results have shown improvement in health outcomes, better quality of life and reduced disease complications. Due to improved self management of diabetes and other chronic conditions, these classes can also help to reduce health care costs.
AgeOptions provides these workshops in trusted and accessible community settings. Classes are led by two leaders trained in the Stanford University model. Fidelity to the class structure and design is a requirement.
Through the workshops, participants learn a variety of self-management tools such as:
- Healthy eating
- Stress management
- Exercise, flexibility and endurance
- Blood sugar monitoring
- Medication management
- Working with your doctor
- Problem solving around the ongoing health condition
- Brainstorming and other techniques
AgeOptions provides two Take Charge of Your Diabetes program options:
Take Charge of Your Diabetes Basic is a free program offered to all interested participants. Participants may currently have diabetes or be pre-diabetic.

Take Charge of Your Diabetes Plus is accredited by the American Association of Diabetes Educators and requires that participants have a referral from a doctor. Medicare will often cover the costs of the program. A registered dietitian leads the classes, provides two individual assessments to collect clinical data related to the participant's diabetes and helps participants identify educational goals to improve outcomes. Outcome data is shared with the referring physician.
