Transition coordinators assist Medicaid-eligible older adults who have been in a long-term care facility six months or longer and are interested in moving back to a community living setting. Assistance may include help locating housing, one-time funding to set up housekeeping and assistive technology and home modifications when needed. Coordinators also continue to work with clients after the transition.
