In-Home Vaccines for People 60 or Older

AgeOptions is working with CIMPAR to administer in-home Flu and COVID-19 Vaccines for people 60+ living in suburban Cook County who need the vaccines brought to their homes. To qualify for an in-home vaccination from AgeOptions, a person must:

  • Live in suburban Cook County (any part of Cook County except the City of Chicago)
  • Be 60 years or older or an adult (18+) with a disability
  • Complete the form at this link: https://forms.office.com/r/vvt3bdsgCG

If someone under 60 needs an in-home vaccination, please refer them to the Cook County Department of Public Health (CCDPH) using the “In-Home Vaccination” form at https://vaccine.cookcountyil.gov/ (see tab in upper right hand corner of the page).

CCDPH offers free rides to and from their vaccine clinics with their Destination Vaccination program.

The federal Centers for Disease Control and Prevention (CDC) continues to provide overall guidance and recommendations for navigating the COVID-19 virus. Please refer to the CDC website for the most up-to-date recommendations.
