Diabetes Self Management

AgeOptions offers Take Charge of Your Diabetes and Tomando Control de su Diabetes as part of the Take Charge of Your Health chronic disease self-management program. Participants attend a series of workshops that provides information and self-management skills for older adults who have diabetes, and their caregivers. AgeOptions also offers an eight-week version of the Take Charge of Your Diabetes Program that includes individualized assessment and consultation with a Registered Dietitian. Economic Security Initiative

This economic security planning program provides options that can help suburban Cook County residents age 55 and older regain their financial footing. The program provides connections to resources to help with finances, including public benefits, legal services, jobs, housing, health and in-home services. AgeOptions was one of eight organizations nationally to receive a grant for the program through the National Council on Aging with support from the Harry and Jeanette Weinberg Foundation and the Retirement Research Foundation.

Hospital Transitions

AgeOptions and a group of community agencies and hospitals in the Bridge Transitional Care Partnership were awarded funding by the Centers for Medicare & Medicaid Services for this program to reduce hospital readmissions among people with Medicare. Through this program, people at high risk of being readmitted after a hospitalization are connected with community supports that help them follow discharge instructions.

Long-Term Support Choices

Person-centered counseling is offered to assist individuals in making long-term support choices.

Suburban Elder Justice Coalition

The goal of this partnership is to meet the needs of older adults who are abused, neglected and/or exploited through communication and collaboration among elder abuse investigators, local law enforcement and the court system.

Senior Health Assistance Program (SHAP)

Community-based sites connect older adults and persons with disabilities to programs that help pay for prescription medications. SHAP sites also provide information, educational programs and individual counseling.

Senior Health Insurance Program (SHIP)

AgeOptions is a Senior Health Insurance Program (SHIP) site, providing health insurance counseling for Medicare beneficiaries and their caregivers. SHIP is sponsored by the Illinois Department of Insurance. Targeting Linguistically and Culturally Isolated Individuals Older adults who speak little to no English can be connected to existing services and programs through translation, referrals and assistance in applying for services and benefits.

Aging and Disability Resource Center (ADRC)

The ADRC offers information to help older adults, caregivers and people with disabilities make decisions about care.
