The Illinois Pathways to Health connects people with programs to increase knowledge and teach skills to better manage chronic conditions, reduce fear and risk of falls, and live happier, healthier, more independent lives.
Our small group, interactive workshops are for individuals living with any chronic condition (such as COPD, heart disease, diabetes), chronic pain or cancer and/or who have a high risk of falling, have experienced – or fear experiencing – a fall. Caregivers, friends, and loved ones are welcome to participate.
We also offer special programming to help caregivers manage stress and difficult emotions and learn to better manage their role.
