Avisery Resources on Dual Eligible Special Needs Plans (D-SNPs)

Illinois’ Medicare-Medicaid Alignment Initiative integrated plans for residents eligible for both Medicare and Medicaid will end on Dec. 31, 2025. In their place, dually eligible individuals can enroll in Dual Eligible Special Needs Plans that will begin operating in Illinois on Jan. 1, 2026.

This resource walks through how to use the Medicare Planfinder to locate and compare D-SNP plans.   It contains screenshots of the Planfinder coupled with tips for interpreting and comparing plan details. Intended for counselors new to using Planfinder and D-SNP  beneficiaries looking for additional support and guidance with Medicare.gov.

D-SNP Consumer Outreach Materials
