In-Home Vaccines & Boosters for People 60 or Older
AgeOptions is working with CIMPAR to administer in-home COVID-19 Vaccine and boosters for people 60+ living in suburban Cook County who need the shot brought to their homes. To qualify for an in-home vaccination or booster from AgeOptions, a person must:
- Live in suburban Cook County (any part of Cook County except the City of Chicago)
- Need an initial COVID-19 vaccine or any CDC recommended booster shot
- Be 60 years or older
- Complete the form at this link: https://forms.office.com/r/vvt3bdsgCG
If someone under 60 needs an in-home vaccination or booster, please refer them to the Cook County Department of Public Health (CCDPH) using the “In-Home Vaccination” form at https://vaccine.cookcountyil.gov/ (see tab in upper right hand corner).

CCDPH offers free rides to and from their vaccine clinics with their Destination Vaccination program.
